Purpose/Goals: 

Community Clinics Project provides integrated primary care, disease management, psychiatric resources, mental health referrals, substance abuse services and linkages to other services for (peri)homeless individuals in the Bangor area.  The Community Clinics Project is funded by the Robert Wood Johnson Foundation with MCPH providing evaluation services.
